The Hospice Admission Process
Admitting a chronically or terminally ill patient into hospice is simpler than most families expect.
A Physician’s Order
Hospice admission requires an order from the patient’s doctor, along with supporting medical documentation establishing a terminal diagnosis with a prognosis of six months or less. Our nurses can assess a patient before an order is written, but an established physician must ultimately write the hospice order and substantiate the diagnosis.
Nurse Assessment & Care Planning
Once the order is submitted, a registered nurse reviews the patient’s medical history and visits the home to evaluate needs, then builds a personalized care program for the patient and family.
Ongoing Care Team Visits
The care schedule varies per patient and may include CNA visits per the care plan, RN assessments and adjustments, and visits from social workers and chaplains as needs evolve.
